Private Pay Rates

Our private pay rates for the semi-private room is $300 a day.

Acceptance of Medicaid

We do accept Medicaid in the building for long-term care.

Medicaid Specialist

We have a Medicaid specialist in the building who works with families interested in and who qualify for Medicaid.

Types of Professionals and Training

Our staff includes Registered Nurses (RN), Licensed Practical Nurses (LPN), and State Tested Nursing Assistants (STNA), with a staff-to-resident ratio of 8:1. Our professionals undergo comprehensive training specific to their roles within our facility to ensure they provide the highest quality of care to our residents.

Resident Safety and Well-Being Measures

We prioritize the safety and well-being of our residents with several measures, such as the use of Wonder Guard in our memory care unit, bed alarms, and securing our facility with coded doors to prevent unauthorized access.

Transition Support and Resources

We offer assistance through our licensed social worker, who coordinates with outside resources to help residents transition smoothly back into the community.

Transportation Services

Our in-house transportation coordinator manages transportation services, which vary depending on insurance coverage. We also provide a Florence Park bus for resident outings and activities.

Dining Experience

Residents enjoy a dining experience that includes breakfast, lunch, and dinner in their stay. An alternative menu is always available at each meal for residents who prefer different options.

Complaint or Issue Resolution

We have a grievance committee dedicated to resolving any issues or complaints from residents within 24 hours.

Communication with Resident Families

Our licensed social worker facilitates care conferences that include residents, families, and any additional requested personnel, such as nursing staff, therapy, activities coordinator, administrator, and dietary staff.

Short-Term or Respite Stays

We offer respite stays at a contracted rate of $300.00 per day.

Pet Policy

While we do not allow pets to stay, family members are welcome to bring friendly pets during visiting hours, provided they have documented vaccination records. Additionally, a friendly cat visits our residents on the smoking patio.

Vaccination Availability

Residents have access to both COVID-19 and flu vaccinations.

Aging in Place

Our facility allows residents to age in place. We have seven private rooms, all currently occupied and with a waitlist. These rooms are allocated primarily based on clinical needs.
