Do you guys currently have a waitlist?

Yes, we do have a waiting list.

Could you share pricing and any other general information?

Our cost is $530.80 per day to pay privately. We also accept Medicaid if there are not funds available to pay privately. We are a 309 bed skilled nursing facility, we offer short term rehab, long term care including dementia/memory care. We also have a 66 bed unit for patients with Huntington’s Disease.

Could you provide an overview of the types of professionals on your staff and the training they undergo at your facility?

We have Doctors, NPs, RNs, LPNs, CNAs, Physical, Occupational and Speech Therapists as well as Dieticians. We provide ongoing skills training for all clinical staff.

Could you tell me about the measures you have in place to ensure the safety and well-being of your residents?

The staff is trained on policies related to safety in the workplace. We have a wanderguard system in place to keep our wandering patients safe. We have fire drills as well as tabletop evacuation drills with our local community.

Do you offer any resources or support services to aid residents' transition into the community?

A social worker assists with the transition to home and ensures all needed medical equipment and services are provided prior to the patient being discharged home or to other care settings.

Do you offer transportation services for residents?

We do not offer transportation but work with ambulette companies for off sight appointments.

Could you tell me a little about your dining experience?

Each of our 40 bed units has their own dining room, we have a direct dining program which means meals are served from the dining room verses being plated in the kitchen and brought up to the units.

What is the process that takes place if a resident has a complaint or an issue they want addressed?

All complaints and concerns are taken seriously and investigated thoroughly. The social worker meets with the resident to gather the information and escalates the complaint to the appropriate discipline. Once a complaint has been investigated the staff meets with the resident to discuss the outcome and steps taken to ensure the incident does not occur again. There is also an Ombudsman Program, which is a separate agency who is also available to meet with the residents.

What is your policy on communication with resident's families? How do you keep them updated about their loved ones' well-being?

We call families with any change in condition and encourage families to call us for updates in between. Many of our families call us daily for updates.

Do you all offer short-term or respite stays? How much does this cost?

We do offer both short-term and respite stays. If a patient is paying privately, it is $530.80 per day, if Medicaid is established, Medicaid cover respite stays.

Are you all a pet-friendly community?

While we do not have any pets living here, pets are welcome to visit once the vet record has been received showing they are up to date on their vaccines.

Are residents able to get Covid vaccination and flu vaccinations in the community?

Yes, we provide the vaccines here. We strongly encourage our residents and staff to receive their vaccinations.

Are residents able to age in place?

Yes, as a skilled nursing facility, we are able to provide custodial, palliative and end of life care.
