Our Current Rates*
- Private Pay Rates (with no Medicaid benefits and / or Medicaid Waiver) range from $4,675 per month for Studios, $5,025 per month for One Bedroom apartments, and $5,885 per month for Double Occupancy (2 persons)
- STUDIO RATES: Medicaid | Waiver rates range from $633 – $1,267 per month for Studio apartments
- ONE BEDROOM: Medicaid | Waiver rates range from $1,267 - $1,606 per month for One Bedroom apartments
- There are additional but optional monthly fees for cable ($34), landline phone ($30) and our emergency alert bracelets ($18)

Important Notes
Please note that we are not able to give exact amounts that one would qualify for until we have a completed application to verify income. Financial assistance programs are available to those who qualify for full Indiana Medicaid and Waiver programs. If you qualify for financial assistance programs, you will need to be FULLY APPROVED by BOTH Indiana Full Medicaid Benefits and you must also qualify for the Indiana Medicaid Waiver program before you can move into Evergreen Village. These are two separate programs that are both needed in order to reside at Evergreen if in need of financial assistance. These financial assistance programs are only available through the Area Agency on Aging in your county of residence or via an Elder Law Attorney. Area 10 Agency on Aging in Ellettsville, IN (serves Monroe & Owen Counties) may be contacted at (812) 876-3383. I highly recommend contacting Area 10 during the application process with Evergreen Village whereas the processing time for both is about equal. (TIP: It’s always best to apply to Evergreen Village AND to also apply for Medicaid at the same time if financial assistance will be needed.)

Overview of Our Staff
We are a fully licensed Assisted Living Community consisting of several types of professionals:
- Licensed Executive Director who is also a Registered Nurse
- A full staff of fully Licensed Nursing professionals (LPN, RN, QMA, and CNAs)
- Dietary department
- Business Office Manager
- Move-In Coordinator who specializes and is Certified in Tax Income and Fair Housing Laws and standards
- Director of Sales & Marketing who is also a Certified Dementia Practitioner and Certified Fair Housing for Seniors Specialist
- A full staff of Licensed Physical, Speech and Occupational Therapists
- Certified Activity Director / Resident Services Coordinator
- Maintenance Director trained in building codes and safety measures
- Licensed Nurse Practitioner in our community three days a week
We have a well-tenured, seasoned staff here at Evergreen Village. All Staff are required to do monthly certified training courses on various topics such as Dementia, Food Safety, Emergency Preparedness, and more.
Safety and Well-being Measures
Safety of our residents is our number one goal! The Indiana State Department of Health & Fire Safety inspections occur annually. Our Maintenance Team checks the building equipment daily and our corporate crew does monthly inspections to ensure all items are in working order and well-maintained. All egress doors are locked from 10 pm – 6 am. Any door that is opened between those hours will alert the nurses and CNA devices. A fire panel is viewable to allow us to immediately pinpoint if and where any smoke is detected. We follow the Indiana State Guidelines for assisted living communities. Cameras are in prime locations throughout the building. Every resident is seen by nursing each and every day to make sure all is well. If a resident leaves our community for the day with family or on their own, they are highly encouraged to sign out at the front desk so that we know where they are in case of an emergency.
Resources for Transition into the Community
We have resident ambassadors & welcoming committees that include both staff and residents! Monthly activity calendars are given to each resident and hand-delivered to their apartments plus posted in various locations around our community.
Transportation Services
Yes! Twice a week there are Resident Trips out and about in our community and beyond (Shopping, dining). Wednesdays we offer to transport our residents to physician appointments locally.
Dining Experience
We offer a daily restaurant-style dining experience for all residents with three full meals per day plus snacks and drinks provided 24 hours a day.
Resident Complaints and Issues
We promote an “open-door” policy. Residents are encouraged to bring any issue they have directly to the administrator. If they prefer, we also have a form for Resident grievances that can be filled out and submitted to either the administrator or to our Corporate office. These forms are available in our Evergreen Resource center on our main floor. We also have monthly Resident Council meetings and a Resident Food Council meeting monthly.
Communication with Residents' Families
We welcome family connections! We highly recommend downloading the “Caremerge” app to stay in touch at all times with your loved one here at Evergreen and stay up to date on upcoming community events. Our Nursing staff, Resident Services, and Executive Director can communicate with each resident’s family through the Caremerge app. Of course, if it’s an emergency, we will pick up the phone and call you!
Short-term or Respite Stays
Our leases are valid for one year and are renewed annually. We are at full occupancy with a waitlist of people that would like to move to Evergreen to live. We could possibly offer respite services if we had availability but at the current time, we do not foresee being able to offer respite service anytime soon. If we did, the rates would reflect our current private pay rates, and one would have to go through the entire application process and nursing assessment, just as all residents do.
Pet-Friendly Community
YES! We love our four-legged friends here at Evergreen! One well-behaved therapy/comfort pet is allowed per resident with physician orders and current vaccine records provided the resident can give proper care needed for the pet and annual vaccines. A non-refundable $500 Pet Deposit is required at Lease signing for pet owners. Fully registered therapy pets are NOT required to pay the deposit, but ALL pets must pass our new Pet assessment prior to admission.
COVID and Flu Vaccinations
In the past, we did offer the COVID vaccines to both residents and staff that wanted them. We do offer an annual Flu vaccine clinic for residents. We do not require these vaccines. If a resident would like to receive the COVID vaccine, they can schedule that with their physician.
Aging in Place
Absolutely! The emphasis here at Evergreen Village at Bloomington is on maintaining health, wellness, dignity, and independence. From the moment a new resident arrives, our team rolls out the red carpet and makes sure they are comfortable. When they need a helping hand, Evergreen Staff is there to offer personal attention to each and every resident. Our Nurses assist with medications, showers, and their activities of daily living. Therapy services are offered full-time Monday through Friday and include Physical, Speech, and Occupational therapies. If, however, our services are not enough as one would start to decline in their health – then outside professionals like Home Health or Hospice are welcome to be here at Evergreen Village to assist our residents with their changing needs.

Please know that we are running on a waitlist, but I do want to advise you that if seriously interested in an apartment at Evergreen, you will need to get on our waitlist as soon as you can. The faster we get the completed application pack and all documents we request, the faster one can move up the list. I will just need the first 3 pages of the packet I can send you filled out, signed, and returned to me as soon as possible to get on the waitlist and the remaining pages of the full application plus all documents requested will need to be turned into us within 10 business days after we receive the initial waitlist. We do not need to meet for you to fill out the application—most people complete the application on their own and collect all the documents and then email it all back to us or drop it off to the attention of Lisa or Cat, our Move-In Coordinator, at the front reception desk. If, however, you have any questions about the application, you are welcome to call us and we'll be happy to help. You will need to reach out to your Area on Aging office in your county of residence—they will guide you through the Medicaid and waiver process. I have the numbers to all county offices in the state if you need help.
A little more about Evergreen Village at Bloomington:
- We have one bedroom and studio apartments, depending on needs / preference.
- Minimum age requirement is 62 years of age.
- Our nursing staff is here 24-hours a day and we assist with medications and showers only.
- We take care of all laundry and offer light housekeeping.
- All meals are prepared for our residents 3x a day – meals are NOT delivered to resident rooms and residents MUST be able to get themselves to our dining room and around our community.
- Activities are ongoing daily with special day trips to shopping, dining, and adventures scheduled weekly.
- We offer Physical, Occupational and Speech Therapy in house Monday - Friday.
- We have a Beauty Salon and a General Store within our community.
- Apartments are unfurnished. We provide a small refrigerator with freezer (approximately 3.5 cubic ft overall) and microwave only.
