Our Team

Our team consists of a variety of highly skilled professionals, including physicians, licensed nurses, certified nursing assistants (CNAs), a social worker, activities coordinator, and trained caregivers. We are a Skilled Nursing Facility (SNF) first and foremost, so we do have an in-house therapy team to assist with physical, occupational, and speech therapy needs. All staff members continue with ongoing education and certification updates to stay current with best practices and industry standards.

Resident Safety and Well-being

Resident safety and well-being are our top priorities. We have a comprehensive safety protocol that includes emergency call systems in all rooms, and regular safety drills for both residents and staff. Additionally, our healthcare team monitors each resident’s health closely to catch any potential concerns early. We also conduct routine safety inspections to ensure our environment is hazard-free and meets all regulatory safety standards.

Transition Assistance

We understand that transitioning into a new living environment can be challenging. Our staff has the resources to assist new residents and make the process as smooth as possible for them. Our activities director and social worker are at the facility during the week and can provide more of that information. We also coordinate with our resident's healthcare providers and create personalized care plans tailored to each individual’s needs.

Transportation Services

Yes, we provide transportation services for our residents. These services include transportation to medical appointments if they are pertaining to why a patient is with us, such as a follow up appointment with their orthopedic surgeon. We have a wheelchair-accessible vehicle to accommodate all residents as well as transportation services that we are contracted with that can provide wheelchair and stretcher transportation if needed. Please note that some appointments will need to be postponed while a patient is with us for skilled nursing.

Meal Options

We offer a variety of meal options tailored to meet dietary restrictions and preferences, prepared by our culinary team. We have a dining room for our residents, and they can also enjoy meals in their rooms if they prefer.

Resolving Concerns or Complaints

If a resident has a concern or complaint, we encourage them to speak directly with our management team. We take all concerns seriously. Concerns will be reviewed by our leadership team, who work collaboratively with the resident and their family to resolve the issue promptly and effectively. We believe in fostering open communication and always encourage feedback to improve our services.

Costs and Insurance

The cost of living in our community varies based on the type of accommodation and the level of care required. Most insurance will cover a portion if not all of a patient's skilled service that they require. This is all based on the residents' needs and also their insurance benefits. If a resident is looking to stay in our long-term care unit, the daily cost is $280 per day and is prorated for the month.

Family Communication

We maintain regular communication with residents’ families to keep them informed about their loved ones’ well-being. Families can expect updates from our care team, including health reports and general updates on their loved one’s activities. We also encourage families to visit and participate in meetings with our staff to stay involved in their care plan. If you would like to discuss your loved one's care plan, an appointment would need to be scheduled with our care team.

Skilled Nursing and Respite Stays

Yes, we offer skilled nursing, and respite stays for those who need temporary care. These stays are ideal for family caregivers who need a break or when a resident is recovering from surgery or illness. Respite stays are usually 5 days, at the cost of our daily long-term care rate which is $280 per day. Skilled stays are authorized by the patient's insurance, or they will qualify with traditional Medicare as long as they have a skilled qualifier, and a 3 consecutive night stay in a hospital.

Pet-Friendly Community

Yes, we are a pet-friendly community! We understand the importance of pets in our residents’ lives and welcome them as part of the family. We do have guidelines to ensure the safety and well-being of all residents and pets in our community. Not all pet visitations will be approved and are on a case-by-case basis.

Waitlist Information

Currently, we do not have a wait list for skilled stays although long term care may incur a wait list depending on the needs of our current residents that are in our skilled unit that may need to transition into our long-term care unit. We encourage prospective residents to reach out to me as early as possible to secure a spot or join the waitlist, if applicable.

Vaccination Availability

Yes, we offer both COVID-19 and flu vaccinations to our residents on-site. We coordinate with local healthcare providers to ensure all residents have access to vaccinations as needed.

Aging in Place

Our community supports aging in place, which means residents can continue to live here even as their care needs change over time. If a patient's care needs exceed our capabilities at any point, we would look to transfer them to a more appropriate facility that would better suit them.
