Essential Care Services
Includes medication management, stand-by assistance or verbal cueing for activities of daily living (ADLs)
Enhanced Care Services
Includes medication management and hands-on assistance with up to 3 activities of daily living (ADLs). ADL services may include bathing, dressing, grooming, feeding, toileting, and other support. Coordination of care services such as hospice, home health, etc.
Comprehensive Care Services
Includes medication management and hands-on assistance with 4 or more activities of daily living (ADLs), and/or ADLs requiring 2 person assistance. Additional support for residents requiring daily redirection to address persistent conditions of anxiety, aggression or exit-seeking. Coordination of care services such as hospice, home health, etc.
Specialty Services
Incontinence Care Program – $400
Mechanical Lift Service – $500
One Time Community Fee – $2,000
