We provide a choice of various floor plans along with delivery of services that support each resident. Services include assistance with activities of daily living, as necessary, including, but not limited to bathing, dressing and grooming. All meals, housekeeping, laundered linen service, maintenance, utilities and social activities are included.

Individual Service Plan

A functional needs assessment will be completed for each resident to determine the service plan that will be carried out upon arrival. The service plan will be reviewed and can be modified as necessary during residency. The ongoing reviews will be the basis for the assessment of additional services to be provided.

• Apartment transfer fee $250.00 one time fee - resident is responsible for moving belongings/furniture

• Pendant Fee $150 for initial & replacement of pendant, if lost or damaged.

• Standardized Med Planners $30.00 one time fee on initial invoice
• Transportation $25.00 fuel fee + $15/hour trip charge - Frankfort/Lexington Area
• Application Fee A deposit of $2000.00 is required to reserve an apartment. This deposit does not apply towards the first month’s rent.

In Pathway

We provide a choice of floor plans ranging from a private suite with full bath and personal closet space to a private deluxe with an added living space in a secure environment. We also offer semi-private suites that can easily accommodate couples for occupancy.

All Inclusive Suite Rates

Services include assistance with activities of daily living, as necessary, including, but not limited to bathing, dressing and grooming. All meals, housekeeping, laundry and linen service, maintenance, utilities and social activities are included. Our Pathway staff deliver individualized approaches to meet each resident’s needs. Base rates are guaranteed for one year from the date of occupancy:

• Private Suite - 237 sf. - $6250/month
  Includes Living/Bedroom Area, Full Bath & Closet
• Private Deluxe Suite - 344 sf. - $6875/month
  Includes Living/Bedroom Area with added sunroom, Full Bath & Closet
• Semi-Private - 344 sf. - $6875/month
  Includes Living/Bedroom Area with added partition for privacy. This partition can also be removed to offer additional space for couples or individuals.

Rental furniture available through Rent a Center in Lexington.
The cost will be the responsibility of the resident.
Daisy Hill will help facilitate the logistics of the arrival of the furniture.
Personal furniture can be used during the short term stay terms.

Application Fee A deposit of $2000 is required to reserve a suite. This deposit does not apply towards the first month’s rent.

At the Villas on Daisy Hill Farm

You have choices, including optional payment selections. You can choose to rent your home with all of the basic services included. You can also choose a life lease with a one-time payment plus a monthly maintenance fee. The choice is yours. Deposit required on either option.

Rental Option

With this option, you choose your home and make a simple monthly payment. The benefits include:

With this option, you choose your home and enjoy all of its benefits with a one-time upfront payment and a monthly maintenance fee. The benefits include:

Application Fee A deposit of one month's rent for a rental or 1% of the purchase price for life lease is required to reserve a Villa.

Overview of Staff and Training

We are a social model with no licensed staff. Our team includes an Executive Director (ED), Resident Services Director (care team falls within), Dining Service Director, Maintenance Director, and Activity Director. Our staff receive 6 hours of dementia care training and 4 hours on other general topics. We utilize RELIAS learning for this platform and conduct bi-monthly in-person meetings for training.

Safety and Well-being Measures

All of our side doors (13) are keypad controlled for entry, and the front door locks at "dark." We partner with Amedisys Home Health Group for resident therapeutic needs and house EmpowerMe Wellness in our building with their staff providing full-time outpatient therapy.

Support for Transitioning Residents

We refer to Moving Services and Concierge Services for transitional logistics. I am pursuing a partnership to offer emotional support/therapeutic services to aid in the emotional transition into long-term care.

Transportation Services

Yes, we do offer transportation services for residents.

Dining Experience

Breakfast is made to order with daily featured items. Lunch is our heaviest meal, and dinner is at 4:30 PM. We also have always available menu items.

Complaint and Issue Resolution

Residents can see the Resident Services Director or ED to address complaints. They may also speak with the Chair of the Resident Council to bring concerns to administration attention.

Family Communication Policy

We have an on-call number for urgent resident concerns after hours and on weekends (my RSD and I rotate this on-call duty). We offer to schedule quarterly service plan (care plan) meetings with each resident/family, or as needed.

Short-term or Respite Stays

Yes, we offer short-term or respite stays at a cost of $175 per day.

Pet Policy

Yes, we are a pet-friendly community.

Vaccinations

Yes, residents are able to get COVID and flu vaccinations in the community. We have a nurse practitioner program in partnership with a local PCP office to administer necessary vaccines.

Aging in Place

Yes, residents are able to age in place, but we do not operate a Skilled Nursing Facility (SNF). Once a resident exceeds our care criteria, we assist with placement in a higher level of care facility. We currently have a waiting list for our Independent Living and Memory Care community, but we do have availability in our Assisted Living community.
