Overview of Staff and Training

We are a social model with no licensed staff. Our team includes an Executive Director (ED), Resident Services Director (care team falls within), Dining Service Director, Maintenance Director, and Activity Director. Our staff receive 6 hours of dementia care training and 4 hours on other general topics. We utilize RELIAS learning for this platform and conduct bi-monthly in-person meetings for training.

Safety and Well-being Measures

All of our side doors (13) are keypad controlled for entry, and the front door locks at "dark." We partner with Amedisys Home Health Group for resident therapeutic needs and house EmpowerMe Wellness in our building with their staff providing full-time outpatient therapy.

Support for Transitioning Residents

We refer to Moving Services and Concierge Services for transitional logistics. I am pursuing a partnership to offer emotional support/therapeutic services to aid in the emotional transition into long-term care.

Transportation Services

Yes, we do offer transportation services for residents.

Dining Experience

Breakfast is made to order with daily featured items. Lunch is our heaviest meal, and dinner is at 4:30 PM. We also have always available menu items.

Complaint and Issue Resolution

Residents can see the Resident Services Director or ED to address complaints. They may also speak with the Chair of the Resident Council to bring concerns to administration attention.

Family Communication Policy

We have an on-call number for urgent resident concerns after hours and on weekends (my RSD and I rotate this on-call duty). We offer to schedule quarterly service plan (care plan) meetings with each resident/family, or as needed.

Short-term or Respite Stays

Yes, we offer short-term or respite stays at a cost of $175 per day.

Pet Policy

Yes, we are a pet-friendly community.

Vaccinations

Yes, residents are able to get COVID and flu vaccinations in the community. We have a nurse practitioner program in partnership with a local PCP office to administer necessary vaccines.

Aging in Place

Yes, residents are able to age in place, but we do not operate a Skilled Nursing Facility (SNF). Once a resident exceeds our care criteria, we assist with placement in a higher level of care facility. We currently have a waiting list for our Independent Living and Memory Care community, but we do have availability in our Assisted Living community.
