Assisted Living Service Fees

In addition to the monthly rental fee, a monthly personal care fee for assisted living services is required based on the amount of care provided. The following steps help to assess your care needs and guide you through the residency process:

1. Reserve an apartment at Croixdale. We will help you through the reservation process and provide you with essential information in preparation for the following steps.
2. Schedule an assisted living care assessment appointment. A nurse will meet with you to complete an initial assessment and help create a personalized care plan. The assessment will include a review of your individualized needs and a recommendation of services that could be provided to meet those needs. Each service is assigned a point value and the assessment will give you an estimated total number of points.* To view the care assessment, scan the QR code or visit Croixdale.org and click on "Resident Documents."
3. Schedule a residency agreement appointment. We will review the residency agreement, payment information and other resources with you. Once the residency agreement process is complete, your move-in date will be confirmed.

Assisted Living Service Fee

In addition to the monthly rental fee, a monthly assisted living services fee is required based on the amount of care provided.

Personal Care Levels

• Personal Care Level I: $3,055/month
  • Prompts and reminders for grooming (no physical assistance)
  • Bathroom assistance (1-3 times daily)
  • Shower assistance (1-3 times weekly)
  • Minimal behavior redirection
  • Dining assistance for snacks, beverages, and light breakfast meals
  • Simple medication management (1-14 meds), providing set-up and administration (1-4 times daily); does not include injections
• Personal Care Level II: $4,170/month
  • Simple or standard dressing and grooming assistance (no transfer assistance)
  • Standard bathroom assistance (4-5 times daily)
  • Shower assistance (4-5 times weekly)
  • Dining assistance with plate set up and preparation to eat
  • Medication management (15+ meds) and administration (5-6 times daily); includes blood sugar monitoring but not including injections
  • Oxygen management
  • Moderate behavior redirection
  • 1-person transfers
• Personal Care Level III: $5,285/month
  • Extensive dressing and grooming assistance (including transfer assistance)
  • Extensive bathroom assistance (6+ times daily)
  • Shower assistance (6+ times weekly)
  • Dining assistance including plate set up and observation
  • Medication management and administration (7+ times daily); includes blood sugar monitoring, insulin injections, and sliding scale management
  • Oxygen management
  • Complex or frequent behavior redirection
  • Mechanical lift transfers

Included in Your Assisted Living Service Fee:

• Laundry (up to 3 loads per week)
• Escort to meals and activities
• Assistance with ted hose, ace wraps or Velcro leg wraps
• Daily reassurance check
• Initial and ongoing nursing assessment
• Scheduled nursing hours and on-call nursing available for staff consultation
• Nurse liaison with health care professionals
• Nurse scheduling of lab draws
• Nurse maintaining clinical record information

To ensure the safety and well-being of our residents, we conduct daily "I'm okay" checks and maintain a secure building environment. Additionally, we offer supportive services tailored to meet the individual needs of each resident.

Support Services for Transitioning Residents

We offer a range of resources to aid residents in transitioning into our community. This includes a 5-day complimentary escort to and from all meals and activities, as well as community tours upon request, to help new residents acclimate comfortably.

Transportation Services

Yes, we provide transportation services for our residents to facilitate their mobility and independence.

Dining Experience

Our dining experience features restaurant-style dining with the flexibility of take-out or tray delivery options, catering to the diverse preferences of our residents.

Addressing Resident Complaints and Issues

We maintain an open-door policy, ensuring that any complaints or issues raised by residents are addressed and resolved promptly.

Communication with Residents' Families

Each resident is assigned their own nurse, who is responsible for communicating any emergencies or significant changes in health to the designated family contact, keeping families informed about their loved ones' well-being.

Short-term or Respite Stays

No, we do not offer short-term or respite stays at our facility.

Pet Policy

Our community is pet-friendly for service pets only, ensuring accessibility for residents who rely on these essential companions.

Vaccination Services

Residents are able to receive annual flu shots within the community, promoting health and well-being.

Aging in Place

Our continuum of care campus allows residents to age in place, with the possibility of changing apartments to receive appropriate care levels as their needs evolve. Currently, we have a two-bedroom assisted living apartment available with no waitlist.
