LEVELS OF CARE

We recognize that individuals require customized personal assistance and health care. For this reason, we offer Levels of Care based on personalized assessments of each resident’s unique preferences and needs. If you are interested in a move to CountryHouse, your loved one will be given a free assessment by our on-site nurse and assigned an appropriate Level of Care. We offer four care levels.

If your loved one is evaluated at a Level 2 of care or lower, you may elect to choose an all-inclusive plan.

COMMUNITY FEE

A community fee of $2,500 secures the room of your choice for up to 15 days. This is due prior to move-in. It is not refundable after 30 days of residency.

A $500 deposit adds you to the waiting list for our community and is applied to your first month’s rent. This deposit is fully refundable if a move does not occur.

OTHER PRICING CONSIDERATIONS

Monthly pricing is for annual lease with the option of a 30-day notice to terminate.

Families are responsible for services not normally provided by CountryHouse, such as personal telephone or salon services.

Overview of Staff Professionals and Training

We have 6 managers that work Monday to Friday at the facility. Our management team includes a Human Resource Manager, Director of Nursing, Maintenance Manager, Sales Manager, Life Enrichment Manager, and Culinary Manager. Additionally, we have a full staff of caregivers and med aides available at all times. Our Executive Director is available 24/7, and there is at least one manager on duty during weekends, usually more. The number of caregivers is based on the needs of our residents.

Safety and Well-being Measures

All doors and gates are secured at all times, and visitors are required to sign in and out. Staff undergoes quarterly safety training on procedures for tornadoes, elopements, fires, etc.

Resources and Support Services for Transition

When possible, we conduct a home visit to meet the resident before they move in to better understand their likes, dislikes, and past activities. We prepare a care package based on this information. Our full-time Life Enrichment Manager and assistant plan activities all day and encourage residents to participate, allowing for a period of adjustment if needed.

Dining Experience

Our facility is known for its amazing food, often catering out our own events. Breakfast is served between 8 and 9 AM, followed by a snack at 10 AM, lunch at noon, a snack at 2 PM, and dinner at 5 PM. We are always happy to provide additional food if a resident is hungry.

Handling Complaints and Issues

Our facility is small enough that any unresolved issues by a manager go directly to the Executive Director. Staff is available from 6 AM – 8 PM, and a caregiver is available 24/7 to contact management in emergencies.

Communication with Residents' Families

Communication with families is customized to their preferences. They can receive updates via calls, texts, emails, and through the Life Loop app to see daily activities. We also strive to send pictures of residents when possible.

Short-term and Respite Stays

We offer both day stays and respite care. Day stays can last up to 8 hours at $125/day, while respite stays cost $275/day (24 hours) and can be of any duration.

Pet-Friendly Policy

We have a dog that visits with the Life Enrichment Director. Family members can bring animals when visiting, but residents cannot keep pets with them.

Availability and Waitlist

We currently have 3 open rooms: one private and two deluxe private rooms, all with large bathrooms.

Covid and Flu Vaccinations

Yes, residents can receive COVID-19 and flu vaccinations in our community.

Memory Care at CountryHouse Residence

CountryHouse Residence provides Memory Care to our residents. In Memory Care, we offer studio apartments that provide our residents and families the option of having a companion suite where your mom would have her own room but share a bathroom with another female resident, or the option of private apartments (Private or Deluxe Private).

Monthly rates at CountryHouse will include room rent, 3 meals a day, snacks and refreshments anytime, housekeeping, laundry, cable, Wi-Fi, local transportation, and any activities your loved one wishes to participate in. On top of the monthly rent, there will be a level of care charge, and this care charge will be determined through a nurse’s assessment.

Personalized Activities Program

At CountryHouse Residence, we offer an amazing, personalized activities program for our residents. Whether it is inside of the building or outside in our community, there is always something your loved one will enjoy. From baking cookies, movie nights, bingo, bonfires in our beautiful courtyard, and live music in our community. We also offer activities that allow our residents to get out of the building, like going on a scenic drive, a movie at the movie theater, or a local baseball game, there is always something being offered.
