We are a one year private pay facility

• Independent 1 bedroom/1 bathroom $3,800
• Independent 2 bedroom/2 bathroom $4,100
• Assisted Living 1 bedroom/1 bathroom $4,800 (PLUS LEVEL OF CARE)
• Assisted Living 2 bedroom/2 bathroom $5,100 (PLUS LEVEL OF CARE)
• Memory Care/CBRF Studio $5,200 (PLUS LEVEL OF CARE)

What is included in your monthly rate???

Memory Care/CBRF:

• All 3 meals are included along with a snack, utilities, 1 basic cable box, Wi-Fi, and a phone jack
• We supply a bed, dresser, and nightstand, but we do encourage you to bring your own personal items!
• If you choose to bring your own bed you can fit up to a queen size bed in your studio room. We just ask that you bring a pillow, comforter, 2 sets of sheets and a mattress pad.
• We supply towels for showering. You would just need to supply a shower curtain and shower rod along with garbage can, laundry basket, personal clothing items that are labeled, toiletries, depends, etc.
• You can hang pictures on the walls; we just ask that you use command strips. You are allowed to hang up curtains along with mounting the TV on the wall using a bracket.

Additional Costs:

• Second person fee $800.00
• Meal Plan added for an independent $350.00 (adds breakfast and dinner daily)
• Meal delivery to a resident’s room $6.00 a day for Assisted Living
• Parking $80.00 a month
• Storage $30.00
• Additional cable box (1 included) $20.00 a month

Overview of Professional Staff and Training

We have trained caregivers, med techs, and CNA’s. Our staff undergoes monthly training and yearly education to keep their skills sharp and knowledge up-to-date.

Measures to Ensure Safety and Well-being of Residents

Each resident's safety concerns are unique, but we have several measures in place, including call lights for immediate assistance, frequent checks based on individual care plans, and the presence of RNs and LPNs in the building from Monday through Friday and on-call during weekends.

Resources and Support Services for Resident Transitions

While we don't have specific transition services, we do offer contacts for movers and organizers. We personally welcome each resident and strive to make them feel at home.

Transportation Services for Residents

We offer a bus service for which residents pay, in addition to resources such as Fox Valley Cab Service and Pumpkin Carriage Services.

Dining Experience

Our meals are homemade, with fresh produce delivered daily from Mercheses in Milwaukee. We serve everything table-side and take pride in our top-notch desserts.

Complaint and Issue Resolution Process

Our door is always open for complaints or issues. We hold a monthly resident council and are available for private meetings with residents and their families to ensure a comfortable experience.

Communication with Resident's Families

We send out a monthly newsletter and post an activities calendar online. Mass emails are sent in case of sickness outbreaks, and we personally contact the first contact for any falls or significant issues.

Short-Term or Respite Stays

We sometimes offer short-term or respite stays, although our rooms are often full with a waitlist.

Pet-Friendly Policy

Pets are allowed to visit as long as we have the vaccine records.

Waitlist Status

Yes, we currently have a waitlist.

Covid and Flu Vaccinations

Our pharmacy conducts immunization clinics for Covid and flu vaccinations.

Aging in Place

Residents can age in place as long as it’s safe for them. We provide services like home health and hospice care to aid in this process.
