Overview of Professionals and Training

Our team includes a Medical Director, Ellen Mcinerney, a Physician, Aleyamma Mathew, and a Nurse Practitioner, Carrie Dana-Evans. We also have 24/7 RNs, LPNs, CMAs, and CNAs who maintain their licenses through ongoing education. An Infection Control Nurse Educator conducts daily rounds to educate staff and ensure proper infection control practices, including weekly "Wisdom Wednesdays" focusing on specific topics.

Safety and Well-being Measures

Upon admission, each resident undergoes a comprehensive assessment to ensure they receive the proper equipment and care tailored to their needs. This includes assessments for fall risks and elopement risks, with personalized plans developed for each resident to ensure their safety and well-being.

Transition and Support Services

Our Social Worker, Brittany Brady, ensures all residents have the resources they need for a safe discharge. We assist in setting up Home Health Services, Meals on Wheels, and other necessary resources to aid residents' transition into the community.

Transportation Services

We collaborate with County Ride, Best Care, and AAA to provide transportation for ambulatory, wheelchair, and stretcher residents, with insurance assistance available depending on the need.

Dining Experience

Our Dietary Director ensures all meals are made from scratch, offering breakfast, lunch, and dinner with a variety of options to cater to all residents. Special themed meals are also organized monthly by our Activities department.

Resident Grievance Process

Residents, families, and staff can file grievances, which are thoroughly investigated. Our Social Worker handles most grievances, ensuring appropriate actions are taken to address and resolve issues.

Communication with Residents' Families

We schedule Care Plan Meetings after admission and quarterly thereafter, or as needed, to keep families informed about their loved ones' well-being. We also offer monthly Resident and Family Council meetings and send out monthly emails to update families on activities and any significant changes.

Short-term or Respite Stays

Our respite stay costs $315 per day with a maximum of 10 days, requiring clinical information from the Primary Care Physician for review.

Pet-Friendly Community

Pets are welcome, with up-to-date vaccination records required to be kept on file.

Vaccination Services

We offer Covid, Flu, and PNA vaccinations in-house, available upon admission or as requested, with records kept for those who decline.

Aging in Place

Our facility accommodates residents of all ages, from those in need of short stay rehab to those in their last days, with a waiting list available for long-term care and memory care units.

Nursing Facility Overview

We are a Nursing facility with a Rehabilitation Unit, a Long-Term Care Unit, and a Dementia Unit. We provide care for those that have had an incident, ended up in the hospital, and require Sub-Acute care prior to returning home. We also provide care for those that are no longer able to care for themselves or do not have anyone to care for them on our Long-Term Care Unit. Our Dementia Unit is for those that meet certain criteria and offers a program that is tailored to keeping them young mentally and physically.
