Nursing Staff
We are staffed with Registered Nurses (RN’s), Licensed Practical Nurses (LPN’s), and State Tested Nursing Assistants (STNA’s). These licensed professionals are required to maintain their licensures by completing educational Continuing Education Units (CEU’s) every two years. Our Administrator is a Licensed Nursing Home Administrator (LNHA) who also must maintain educational CEU’s every two years. I am a Licensed Social Worker (LSW), and I too need to obtain educational CEU’s biennially. We also have a Nurse Practitioner on staff who is required to maintain educational licensure every two years.
Resident Safety
To ensure the safety of our residents, we complete an elopement assessment upon admission to determine if they are at risk of leaving the building. If they are at risk, we provide a wander guard to the resident, which alarms staff if they come within 15 feet of an exit.
Resources for Discharge
In preparation for a guest’s discharge, I meet with guests and their families to determine what resources, services, community support, and Durable Medical Equipment might benefit them to ensure a safe transition to home. These supportive services include home care, skilled nursing services, aide service, Physical and Occupational therapy services, home-delivered meals, support with acquiring housing, furniture, and assistance with transportation.
Transportation Services
We offer transportation services for guests who have qualifying follow-up medical appointments. This service is provided at no additional charge to the guest.
Dining Experience
We have a Resident Centered Dining Experience, which includes providing our guests with a menu every morning with the meal options for the day. Our guests can choose which items they would prefer for their dining experience.
Issue Resolution
If a guest has an issue or concern, the staff has been trained to complete a Concern Form detailing the issue needing to be addressed. This form is forwarded to the appropriate Department to be addressed with the guest. These forms are completed and given to the facility Administrator for review.
Family Communication
We communicate with families daily to keep them updated on their loved one’s well-being. I conduct care conferences every three months with guests and families to review their current status, medications, quality of care, resident preferences, length of stay, and discharge potential.
Respite Stays
We offer short-term respite stays. For qualifying Medicaid recipients, they have 30 days per calendar year to utilize for respite stays. For private pay guests, we offer respite stays in a semi-private room for $400.00 per day or $450.00 per day for a private room experience.
Pet-Friendly Community
We are a pet-friendly community. We have a facility golden doodle named PAL who visits our facility weekly. We encourage families to bring in their pets to visit our guests! We ask that a shot and immunization record be provided prior to the pet’s visit.
Vaccinations
Our guests are offered Covid vaccinations as well as Flu, Pneumonia, and RSV vaccinations.
Aging in Place
Our guests are able to age in place. We embrace every stage of life and ensure quality care and psychosocial well-being for every guest.
