Concordia of Monroeville Long Term Care

Long Term Care is provided on a Skilled Nursing unit and is not covered by your primary health insurance. LTC services are paid either privately or by Medical Assistance. Our Medical Assistance specialist can meet with you to determine if you qualify for Medical Assistance coverage. If you do not currently qualify you may have to "spend down" some assets before Medical will cover your stay.

Private Pay Room Rates effective 7/1/20:

• Semi-private $315
• Private $335

Personal Care

Personal Care is private pay and not covered under your primary health insurance. If you have a Long Term Care policy, it may cover a portion of your daily rate for personal care.

The following daily rate changes will be effective on January 1, 2024:

Personal Care

• Private Standard $153.00
• Private Large $165.00
• Suite Private $180.00
• Semi Private Suite $128.00
• Semi-Private $120.00

Levels of Care

• LOC 2 $25.00
• LOC 3 $35.00
• LOC 4 $45.00
• LOC 5 $55.00
• LOC 6 $63.00

Long Term Care Waitlist Information

We do have a waitlist for our Long Term Care level of care – in order to be placed on the waiting list, we will need a completed financial application and some clinical information to be sure we can medically manage the person’s needs. I have attached an application in case you wish to be added to it.

Personal Care/Assisted Living Availability

We do not have a waiting list for our Personal Care/Assisted Living area. This is paid for by the resident/family 100% and we have both private and semi-private rooms available. The same financial application and medical information will be needed in order to assess the level of care.
