Primary Care Physicians
Every resident is assigned a primary care physician who is a MD. The doctor also has a nurse practitioner who provides additional visits to our residents. We recently added a new physician and NP this past week who will oversee all new admissions. This will be beneficial because their case load will be smaller in the beginning.Nursing Oversight
We have a director of nursing and an assistant director of nursing/infection prevention almost overseeing all nursing activity. Every unit has a unit manager. Depending upon the census on each unit we will have either 2 or 3 nurses who are either LPN or RN. Our staffing coordinator assured that there are enough CNAs on each unit so that there is a 1:8 ratio during the 7-3 shift, 1:10 on the 3-11 shift, and 1:12 on the 11-7 shift. We also have an RN on-site 24/7. All of our staff are licensed, and HR makes sure that their licenses are up to date.Personalized Care Plan
Every resident has a personalized care plan. This care plan is updated as needed based on events that occur. Our residents start off with a baseline care plan derived from initial assessments upon admission from nursing and our therapy department. These assessments determine what level of assistance is needed, mobility levels, specific wheelchairs, etc. If an incident occurs, such as a fall, we update the care plan and implement fall interventions (i.e., floor mats, low bed, etc).Community Transition Support
I am assuming that your third question is referring to returning to the community upon discharge from our facility. We have two licensed social workers in our facility. They assist our residents and equip them with resources for a smooth transition back to the community. We are very conscientious that every discharge should be a safe discharge. The social workers will set our residents up with hospital beds, home health agencies, etc., whatever is needed for that specific resident.Transportation
Transportation is subject to the resident's insurance and the need for transportation. There are times that the facility will pay for transportation and other times that we will request the family to pay. As far as I know, Medicaid is the only insurance that fully covers transportation.Dining Experience
We have a dining room on our second floor. Residents are encouraged to attend the meals in the dining room to create a social atmosphere. Those who do not want to join the dining room get served their meals in their rooms. There is music playing in the dining room and the meals are served restaurant style as opposed to a tray placed at the bedside. There is a menu to choose from. Lunch and dinner have two options to choose from daily. Other items such as soup, juice, yogurt, and sandwiches can also be requested to accompany your meal.Grievance Procedure
One of our social workers is appointed as our grievance officer. If a resident has a complaint, it should be reported to any one of our staff which will then be brought to the grievance officer's attention. She will then proceed to investigate the matter (gather statements, substantiate, etc.) and ultimately reach an intervention for the grievance. We strive to have any grievance brought to her attention resolved within 48 hours of notification.Resident Profiles and Care Conferences
Every resident has a profile on our computer system, PCC. Part of that profile is a list of contacts that the resident chooses. The numbers are listed in order of priority. If there are any nursing related developments, we will reach out to the primary contact and move down the list until we contact someone. Our staff will not discuss resident-related matters with anyone who is not listed on the contact list. In addition to updates on incidents that occur, we have scheduled care conference meetings. This is a time for the resident and family to meet with our nursing, therapy, and social worker to discuss various issues.Respite Stays
We do offer respite stays and the cost is $325 per day.Pets
Please clarify your question about pets. Are you referring to residents having pets during their stay or families bringing pets to visit their loved ones? Any pet that comes to the facility would require proof of vaccination against rabies. I have not come across a resident having a pet with them throughout their stay. I will ask our administrator for her thoughts on this matter.Vaccinations
We do offer vaccinations for our residents.Aging in Place
Please clarify what you mean by "age in place." We are a nursing facility as opposed to an assisted living facility. Our residents are typically unable to care for themselves and will stay until they are capable of doing so. We often discharge to assisted living facilities or independent living facilities.Conclusion
