Community Details

Community Details

This detailed information was received through an email from the Admissions Director at Clarksburg Nursing And Rehabilitation Center via email on 2024-08-07.

Staff

We have a wide variety of staff in our building including housekeeping, laundry, maintenance, nurse aides, licensed practical nurses, registered nurses, wound care nurses, a nurse practitioner, and a physician (Our physician comes once a week, but is available by phone at all times). A business office manager, medical records, and the list goes on! All of these folks complete on the job training upon hire and they also have monthly training courses they are to complete each month as required by the company.

24/7 Care

We provide 24/7 care, which means there is always someone on a hallway providing care and supervising our residents to ensure their safety. Our nursing team also holds frequent meetings to ensure resident safety, as well as measures we should take to personalize safety measures for different residents. Our team also completes assessments with residents, or their resident representatives, monthly to ensure they are happy with their care. We also hold a resident council meeting for our residents to bring up any topics they would like to discuss whether that be food, activities, nursing care, etc.

Transportation

We do offer transportation for our residents! We have a van onsite with a driver and CNA that will accompany a resident to any appointments, trips to the bank, etc. In the case our van is busy, or if a resident may need transportation by a stretcher, we have a designated person to set up transportation with local ambulance companies who will accompany a resident during their appointments.

Dining

We feature 2 dining rooms in our facility, an upstairs dining room, as well as a downstairs dining room. Residents receive 3 meals a day, with alternate options for each meal. Our food service director will personalize certain meals if a resident prefers one thing over another, and they will also ensure all diet restrictions are met. We also have a pantry on each floor where residents can store their own snacks (if they wish), but we also keep snacks and drinks readily available in there at all times.

Grievances

If a resident has a complaint, they may file what we call a grievance, with anyone. However, typically our social worker will file the grievance for the resident. When a complaint is made, it is brought up to all upper management and we do our very best to complete the request and/or alleviate an issue a resident may be having. Resident’s family and friends can also file a grievance and it will also be brought up to management and we will ensure it is handled in a timely manner.

Cost

As far as cost goes, there are several ways for payment in the facility. The most common is going through someone’s insurance. We have our own intake team that will verify any and all insurances someone may have and we will submit for an authorization from said persons insurance, so they can be covered by their insurance company. Another way to pay would be “Private Pay” which is where the resident (or family member/friend) can pay out of pocket for the residents stay here. The cost of that is $411 per day, which comes out around $12,000 a month.

Communication

When it comes to communication we will call a family member or friend at the request of the residents at any time, or if we feel there is something that needs reported back to the family, etc. Anytime there is a big change with the residents whether that be a medication change, a behavior change, a fall, etc., an emergency contact/MPOA/Resident representative will be notified. We encourage family members to call anytime they like as well, as we will answer any questions (as long as family member is on a HIPPA release) and will connect you with your family member here. Residents can also get landline phones in their rooms through Frontier Communications, but family/resident will be responsible for that bill.

Short Term/Respite Stays

We do offer short term/respite stays here, if it is a true respite stay, sometimes insurance may cover that or we will ask for someone to pay privately which is $411 a day. I would check with your family members insurance provider as they may offer to cover respite stays.

Pet Policy

We are a pet friendly building, we allow pets to visit as long as they are up to date on all vaccinations. We do not allow pets to live in the building however.

Waitlists

As far as waitlists go, I have a few folks on there now but we tend to be able to get them in pretty quick if we can receive all the necessary paperwork in a timely manner.

Vaccinations

Residents are able to receive Covid and Flu vaccines here in the building, we do not offer vaccinations to outside individuals.

Aging in Place

Residents are able to age in place, we will develop their care throughout their stay here and we are constantly ensuring they are receiving care that is adequate with any diagnoses. We have many residents who have been with us for years and we have provided them with care through all stages of their life.
