Chesterbrook Residences

Introduction

Apologies for the delay. Chesterbrook is a pet friendly community with licensed nurses and CNAs as part of our team. They are headed by our Director of Nursing that has been here for 17 years since Chesterbrook originally opened. She and our Assistant Director of Nursing both provide the training for the clinical team.

24/7 Nursing Care

We have round the clock, 24/7 nursing on site to ensure the safety of our residents. Our DON and ADON will arrange and hold vaccination clinics in-house as needed.

Resident Services Coordinator

We also have a licensed social worker on staff that serves as our Resident Services Coordinator to help with residents transitioning into the community, as well as handle any types of issues the residents may have. Our RSC organizes care plan meetings for the residents and families with the appropriate administrative staff to update about the well-being of our residents and discuss necessary changes we all may need to make for the benefit of the resident.

Aging in Place

Currently, residents are able to age in place here at Chesterbrook with certain exceptions since we are not a licensed nursing home. This will be something to discuss with our DON.

Respite Care Services

Chesterbrook does offer respite care services from 30-90 days for a fee that ranges from $260 to $360, pending room type and Level of Care assessed. Three meals a day in our dining room is included in all our pricing. The meals are served buffet style and service is available to residents who are not able to.

Availability and Housing

We currently only have a waitlist for our subsidized housing voucher units but not for our market rate availability.

Transportation Services

Transportation services are offered about twice a week in a group setting for errands. Individual transportation can be arranged with the front desk.
