Our Pricing Model
Our pricing model ensures that residents and their families are charged only for the necessary and desired care at the time it is needed. The determination of care levels is based on a comprehensive assessment that considers the residents’ needs, preferences, and care plan. This innovative assessment tool offers an impartial evaluation of each resident, leading to the creation of a personalized care plan. Overview of Staff and Training at Anthem Memory Care
Anthem Memory Care is a small organization with 25 memory care communities around the country. Our focus is memory care only, so all of our staff are dementia trained and certified by the Alzheimer's Association—that includes kitchen staff, housekeeping...everyone. Our staff also receives ongoing caregiving and dementia training. Our Clinical Director is an RN and she is supported by our Assistant Clinical Services Director who is an LPN. Our Executive Director has 25+ years of experience as a health care administrator. All members of our leadership and front-line team receive ongoing training and support from our home office. Our team is also supported by rounding physicians, hospice providers, and HomeHealth. PT, OT, and Speech Therapy is offered in-house by our partner Legacy Health Care 5 days a week.Measures to Ensure Safety and Well-being of Residents
Like all licensed assisted living communities, we are required to meet all State Regulations for health and safety. You are welcome to look at our State Survey results. Our Environmental Services Director is responsible for the safety of our spaces. If you choose to visit us, I can show you how we handle fire safety as well as elopement prevention. We are a secure environment with an internal courtyard that allows our residents to spend time outside where there is no risk of elopement. Our Clinical leaders and staff provide oversight and care to our residents ensuring that they are cared for and safe. Suites have the Vigil System installed alerting care staff when a resident is moving in their room. This system can be set up specifically for the resident ensuring that staff can respond to those who need support in their rooms. Falls are always a risk for those with dementia, so we do all we can to prevent them, but are responsive when they happen. There are a myriad of regulations that we are accountable to regarding any incidents that may occur. Documentation is required and essential and we take it very seriously.Resources to Aid Residents' Transition
We fully support families through the entire move-in process. My role is to walk families through the whole process, assisting with all paperwork, planning what to bring and referring movers when that is needed. We also help families set up all third-party services including rounding physicians, therapies, foot care, dental, etc. We also have a caregiver support group as well as educational webinars with Dr. Tam Cummings who is our dementia expert.Transportation Services
We do not offer transportation services for medical appointments. We offer rounding provider services in the community as it is hard on our residents to go out to doctors' offices. We DO provide group transportation for life-enriching experiences as part of our robust Life Engagement Program called Pathways of Purpose.Dining Experience
We provide three meals a day and three scheduled snacks, as well as snacks and drinks available anytime. Our meals are chef-made. We do not utilize a menu, rather providing the same meal for all residents while still accommodating for some special dietary restrictions and needs. Our meals can be prepared for those who need mechanical foods, finger foods, or soft foods. We are constantly pushing liquids and offering sustenance as our residents often struggle to get enough calories. I can honestly say our food is fantastic with our Chef Elena's creations from daily soups, comfort food, healthy snacks, and home-baked pastries.Addressing Resident Complaints or Issues
We always want to hear from families right away if they have any concerns. We encourage them to let our Clinical Services Director know immediately if they have concerns or questions about care so that we can address them. Our Executive Director's door is always open to hear any concerns as well, whether it be about dining, care, rooms, staff, etc. We appreciate hearing concerns right away via email or in person. We also regularly schedule care conferences with family members, our team, and third-party providers so that we can address changes in condition or any challenges that are arising with care.Communication with Residents' Families
Families are encouraged to visit often and invited to call the front desk to ask about how their loved one is doing. We are always happy to share updates. If we are seeing changes in condition or behavior that are concerning, we reach out to the POA immediately. We also reach out to POA whenever any kind of incident occurs such as a fall.Short-term or Respite Stays
We do not offer respite stays at this time.Pet Policy
We have visiting pets almost daily, but we do not allow our residents to keep pets.Waitlist Status
We do not currently have a waitlist.Covid and Flu Vaccinations
Yes, residents are able to get Covid vaccination and flu vaccinations in the community.Aging in Place
Yes. As our residents go through the inevitable decline brought on by dementia, we provide supports including two-person assistance, mechanical lift assistance, and bring in hospice partners so that they can remain in our community throughout their journey whenever possible. In my time here, all of our residents have been able to transition in our community surrounded by family and loved ones. For pricing & availability call (866) 204-9238
