Serenity Garden Program

CASTLE SENIOR LIVING at Forest Hills also has a program dedicated to Alzheimer’s and memory impairment care. The Serenity Garden is an entire floor dedicated and specially designed to meet the needs of individuals affected by Alzheimer’s disease and memory impairment offering personal care, recreation, socialization, and supportive care in a warm and safe environment.

The monthly rate in the Serenity Garden is $230 per day for private accommodations and supportive care. Enhanced Level of Care in the Serenity Garden is $20 a day.

Choosing CASTLE SENIOR LIVING at Forest Hills

Prior to moving in to one of our three buildings, we require a $300 refundable deposit, which is applied to the first month’s rent at the time of move in.

Community Fee: $4,000
A community fee is required at the time of move-in. This one-time, non-refundable fee is used to help offset the cost associated with the furnishing and upkeep of our common areas. As well as the care of our courtyard and landscaping. If a resident moves-out within the first 30 days of occupancy, this fee will be refunded on a pro-rated basis.

TIERED OR LEVEL OF CARE FEE ARRANGEMENTS

Supportive Care Living – Level One - $700 Per Month
Includes all Basic Care Services, plus ...
• Reminders and monitoring with grooming including hair, teeth brushing, shaving, etc.
• Daily cueing and monitoring with dressing, clothes selection, orientation
• General cueing for activities of daily living
• Cueing and reminding with showering
• Encouraged to attend social activities
• Reminders to attend mealtimes

Supportive Care Living – Level Two - $900 Per Month
Includes all Basic Care Services and Level One Supportive Care, plus ...
• Supervision with some assistance with showering, three times per week
• Cueing / reminding some assistance with dressing, clothes selection, orientation
• Cueing / reminding some assistance with grooming including reminders to
use incontinence products
• Reminders / Orientation with meals, general nutrition, and activities
• Cueing / reminding some assistance to and from meals and activities

Supportive Care Living – Level Three - $1100 Per Month
Includes all Basic Care Services and Level One and Level Two Supportive Care, plus
...
• Hands on assistance with grooming including hair, teeth brushing, shaving, etc.
• Hands on assistance with dressing, clothes selection, orientation
• Including additional laundry services and additional showering
• Hands on assistance with showering, three times per week
• Escorting resident to and from meals and activities

Medication Management - $618 Per Month
Medication Management includes medication acquisition, storage and disposal,
assistance with self-administration of medication. CASTLE SENIOR LIVING at Forest Hills Wellness Center nurses can administer medications requiring an alternate route of administration (e.g., injectables, eye drops). The New York State Department of Health limits non-oral medications to medications that a licensed health care provider can customarily train a patient or his / her family to administer. This fee does not include medications or supplies.

Incontinence Management - $420 Per Month
If a resident requires assistance due to management bowel or bladder incontinence,
CASTLE SENIOR LIVING at Forest Hills Wellness Center staff can assist with changing bedding as needed, changing clothing as needed toileting or changing the resident every two hours, and the ordering of incontinence supplies if the resident / family desires. The fee does not include cost of incontinence supplies or incontinence bedding supplies.
