Brookestone Meadows Services

We offer short term rehab and long term care here at Brookestone Meadows (we don’t offer independent or assisted living). Currently, we have one female long term care bed available. It is a companion standard room, meaning it would be a shared room. Unfortunately, we don’t have a wait list. If we don’t have availability at the time, we ask that families check back periodically.

Rate Information

I have included our rate sheet for your information. Our room rates will be increasing by $20 starting April 1, 2024. We do offer transportation during the week, there is a charge of $21 one way for our drivers to take residents to appointments.

Pet Friendly and Vaccinations

We are a pet friendly facility and yes, residents can get their vaccinations here at the facility.

Resident Care and Safety

Residents can age in place. However, if they have behaviors or are an elopement risk, we may have to work with family on finding a more appropriate setting for them. Regarding our floor staff, we have RNs, LPNs, Med Aids, and CNs. We have a variety of safety measures in place to keep residents safe. We do regular trainings with our staff to ensure everyone is prepared if an emergency occurs.

Meals and Activities

We offer three meals a day with snacks in between and have several activities for residents to attend during the week.

Social Work Support

We have 4 social workers on staff that help with residents’ psychosocial needs and address any issues that may arise.
