- Please note: The local County Welfare Agency (i.e. Bergen County Board of Social Services) may determine that a resident on MLTSS has cost share, meaning their income exceeds the monthly room and board rate combined with the personal needs allowance rate. Residents with responsibility for a cost share will receive a PR2 indicating the amount of the cost share that must be collected by the facility and how it was calculated.

Staff Availability
On staff we have aids and nurses that are here 7 days/week, 24/7. We also have a nurse practitioner, podiatrist, psychiatrist, DR, social worker, but they are not here 24/7.
Security Measures
Our doors remain closed at all times and you need a code to get in and out of every entrance so that no resident can wander or get out by themselves. They would need a staff member to get out of the building if they wanted to go on walks and we require them to have another resident go with them or an aide so they are not alone in case something happens. No resident is allowed a car on sight either. We also have cameras throughout the hallways to ensure the safety of the residents.
Dietary Care
When it comes to their diet, the kitchen staff follows what our director of nursing allows the resident to have following their special dietary needs such as low sodium or no sugar.
Therapy and Support
The transition can be difficult for any resident so we do offer group therapy that I conduct with my intern here. We also offer individual therapy if needed and wanted. We try to be around more for the new residents so that they can adjust with the new schedule, new residents, and new environment. Since I am the only social worker on staff it can be difficult to have consistent individual sessions because I get pulled in other directions, but we will have the option to have consistent individual therapy with an outside company starting 12/1.
Transportation and Outings
We are a grant funded facility and do not have a bus or large van for transportation, but there are a few staff members including myself that will bring residents on an outing such as lunches or shopping.
Dining Experience
Our kitchen serves breakfast, lunch, dinner, and snacks everyday. We have two dining rooms: one on the first floor and one on the second. The resident has the option to go to either and is served as if you were in a restaurant. A server will come up to them and ask them what they want from the menu that was given to them 24 hours prior or if they chose to have an alternate option because they do not want what's on the main menu.
Resident Council Meetings
We have a resident council meeting 1x/month. Where the residents discuss what they want to improve, want they want more/less of including all/any issues and the chef of the facility asks what they want or don't want and what he can improve on as well.
Communication and Feedback
The resident is also welcome to come to me when I am here Mon.-Fri. 9-5 or to our administrator Jorge or our office manager Dee or our Director of Nursing Lisa. We want to know what is displeasing the resident so that we can fix it or find some solution as soon as possible. I also want the families to be involved and if they need to call me or email me with complaints or issues that is welcomed as well.
Cost of Living
The cost of living in this facility is based on a sliding scale. If you are paying privately and not with Medicaid then it will be from $4200-$7000, depending on the room and the funds the potential resident has. If the potential resident has Medicaid MLTSS then it comes out of their social security and Medicaid is billed the rest. Everything is included in the price given such as meals, housekeeping, laundry service, medication management, having aids/nurses, etc. The only thing that is extra would be to have cable which is $42 extra a month. We do not provide TVs or mini fridges in the room if a resident requires that. We do offer towels, sheets, pillows, beds, dressers, an armoire, toilet paper, paper towels, but the resident will need to provide their own toiletries and laundry basket.
Family Involvement
We want the families of our residents to be as involved as possible. If they want to visit we only ask that they let us know 24 hours prior so that we can have you on a list about coming to see them in our nurses office. We don't want people to just show up because they could be anyone and we have to make sure the resident is safe with that person and they are allowed to be seen by them. As I mentioned before my door is always open so please feel free to contact me if you have any questions or concerns about your loved one. If the resident has a phone then they can call or text whenever they please. We do have some residents who do not have phones so I can schedule them a day to make phone calls on a specific day/time a week or if it's a spontaneous call they can just let me know and I can put some time aside to use my office phone for them. We also have had and want to do this more frequently where we have family nights and our executive director will attend to let you know what new things are occurring, how you can be more involved, to answer any questions you may have, and to get to know the staff a little more.
Respite Stays and Pet Policy
We do not offer respite stays at this facility for Medicaid because of the rules, regulations, and paperwork, We may be able to offer it for private pay depending on the length of stay. We usually do not allow residents to bring pets, however we have made exceptions in the past so it would depend We do have two therapy dogs that come here for the residents 1x/week.
Admissions and Waiting List
As of today, we do have a waiting list. We do not have any more male beds at this moment, but things can change. We have only one room left for a female bed, but have others interested in the room. It is first come first serve so as long as we have all the paperwork required, have assessed the individual, and they are approved because we feel we can care for them then we can move forward with admitting them.
