Overview of Professionals on Staff and Training at Our Facility

We hire Registered Nurses (RN's), Licensed Vocational Nurses (LVN's), Certified Nurse Aides (CNA's), Physical Therapists, Speech Therapists, Occupational Therapists, Certified Occupational Therapy Assistants (COTA’s), and Therapy Managers. Our Department Heads include Administrator, Director of Nursing, Assistant Directors of Nursing (2), Social Worker, MDS Nurses, Activity Director, Dietary Manager, Wound Care Nurse, Wound Care Doctor, Physicians along with their Nurse Practitioners, Human Resources, Business Office Manager, Therapy Manager, Maintenance Director, Admissions Director, Marketing Director, Laundry and Housekeeping Supervisor, and Receptionist. New employees undergo an orientation program, and we hold periodic inservices. Normally, our nurses and Assistant Director of Nursing (ADON) conduct the training for new employees.

Safety and Well-Being Measures for Residents

We prioritize the safety of our residents with several measures. All exterior doors have controlled access. We conduct periodic fire drills and have sprinkler systems in place.

Resources and Support Services for Resident Transition

Our social worker assists residents in transitioning back to home, Assisted Living, Independent Living or Long-Term Care (LTC). We also arrange for home health services and necessary equipment based on the discharge plan and needs of the resident.

Transportation Services

We currently have 2 wheelchair-accessible vans, and our van driver is a licensed CNA.

Dining Experience

Our dining experience features a choice menu with substitutes available if a resident does not prefer the main choice. We also have an anytime menu. We offer home-cooked meals and additional snacks during non-mealtimes. Our goal is to meet the nutritional needs of each resident to the best of our ability.

Resident Complaint or Issue Resolution Process

We have a social worker and a department head for each department to address any concerns that may arise from residents.

Communication with Residents' Families

We hold care plan meetings 10-14 days after a resident's admission and quarterly thereafter. Both the Director of Nursing (DON) and the Administrator maintain an open-door policy for any concerns.

Short-Term and Respite Stays

We offer short-term and respite stays. The cost is $250 per day for a private room and $195 per day for a semi-private room. We cover diapers but not pull-ups. Our pharmacy honors any drug plan the resident has.

Pet-Friendly Policy

Yes, we are a pet-friendly community. Pets can visit, but their shot records must be provided to our receptionist.

Waitlist Information

The only waitlist we have is for Medicaid or pending Medicaid residents.

Covid and Flu Vaccinations

Yes, residents can receive Covid and flu vaccinations within the community.

Aging in Place

Yes, residents are able to age in place. We are licensed as a skilled nursing facility and offer long-term care and hospice services as needed for end-of-life care.
