ASSISTED LIVING RENTAL INFORMATION

Bloom at German Church provides services to residents who need a wide range of assistance. Some residents are independent and require only a small amount of assistance with day-to-day activities. Other residents may need assistance with most activities of daily living. Our Wellness Care Team conducts complimentary assessments of each resident upon move-in and regularly thereafter. The assessments allow you the opportunity to inform the staff what services you desire on a daily basis. It also provides the staff with an opportunity to discuss these services with you, determine the level of services and care that is appropriate and develop a personalized care plan. We individualize our care plans recognizing that each person is unique. Our apartments are rented month-to-month making it a smart financial option. The monthly rent includes our state-of-the-art amenities, basic personalized services and life enrichment program.

Care Levels

• Level 1 $475.00 / month additional

Respite Rate

Residents are welcome to stay with us for a short period of time at the rate of $175.00 per day. All amenities, services and activities are included in the rate. Respite rooms are fully furnished.

Adult Day Care Rate

Residents are welcome to enjoy our Adult Day Care Program Monday – Friday from 7:00am – 6:00pm at the rate of $12.00 per hour. There is a 4 hour minimum and 10 hour maximum. All amenities, services and activities are included in the rate.

Our Staff and Services

We have LPNs (Licensed Practical Nurse), QMAs (Qualified Medical Assistant), CNAs (Certified Nurse Assistant), and a licensed Social Worker. All are licensed by the state. They are in the community 24/7.

Security Measures

We have codes on all doors as well as alarms.

Social and Medical Support

We have a Social Worker on staff and we have a Psychiatrist who meets with all new residents. We have residents that welcome each new person and shows them around and helps them.

Transportation Services

Yes, we have a bus that takes the residents to doctor appointments, shopping, to the bank, and out to eat.

Meals and Nutrition

We provide three meals a day. Breakfast is at 8:00 am, lunch is at 12:00 pm, and dinner is at 5:00 pm. Our menus are approved by a dietitian. We offer an alternate menu if someone does not like what is being served.

Communication with Family

The ask that the resident or family member talks to the Director of nursing or the Executive Director.

We keep the family informed if someone falls, or has any other issues. We have quarterly care plan meetings with the family to discuss any changes, if any, in the resident.

Pricing Information

I will enclose our price list for people paying privately. If someone has Medicaid and the Medicaid Waiver, the state sets the price of $943.00 a month. If they want cable that is $45.00 a month.

Pet Policy

Yes, as long as the resident can take care of the pet themselves.

Healthcare Services

We have CVS come in every October to give the shots. If they want one at another time, the family will have to take them to get the shot.

End-of-Life Care

Yes. We try to have our community be the last place a person would have to move to. We have hospice companies available. The only time a person would have to be discharged from our community is if they become exit seeking, or a danger to themselves or others, or they become medically unsafe for us to care for (needs skilled nursing care). I currently have a small wait list.
