ASSISTED LIVING RENTAL INFORMATION

Bloom at Eagle Creek provides services to residents who need a wide range of assistance. Some residents are independent and require only a small amount of assistance with day-to-day activities. Other residents may need assistance with most activities of daily living. Our Wellness Care Team conducts complimentary assessments of each resident upon move-in and regularly thereafter. The assessments allow you the opportunity to inform the staff what services you desire on a daily basis. It also provides the staff with an opportunity to discuss these services with you, determine the level of services and care that is appropriate and develop a personalized care plan. We individualize our care plans recognizing that each person is unique. Our apartments are rented month-to-month making it a smart financial option. The monthly rent includes our state-of-the-art amenities, basic personalized services and life enrichment program.

Respite Rate

Residents are welcome to stay with us for a short period of time at the rate of $175.00 per day. All amenities, services and activities are included in the rate. Respite rooms are fully furnished.

Overview on Staff and Training

Our nursing staff includes QMA, CNA, LPN, and RN professionals. We conduct background checks and verify licensure to ensure compliance. Their certified specialty is also regulated by the State of Indiana guidelines.

Safety and Well-Being Measures

Residents have their own key to their apartments. We retain access for emergency situations. All doors lock at 7 pm, with a keycode required for entry. A Ring Camera monitors the main entrance.

Support Services for Community Transition

We are a Medicaid and Medicaid waiver community. Residents with monthly earnings below $2600 and no other assets may qualify. Families assist with applications before their loved one moves in.

Transportation Services

We organize shopping and entertainment outings via our activity department. We also provide transport to doctor's appointments if families cannot. For ongoing medical appointments, prior arrangements must be made.

Dining Experience

Our facility offers restaurant-style dining with special options for lunch and dinner. There is also an "always available" menu. Breakfast, lunch, and dinner are included in rent.

Complaint and Issue Resolution Process

Residents can speak to a manager about any complaints. We conduct investigations and provide immediate responses when necessary. For non-urgent issues, a binder is available for requests, along with a suggestion box.

Communication with Families

The nursing team holds care plan meetings with families and residents. Daily nursing meetings address concerns promptly. Information is only shared with the designated point of contact, in compliance with HIPAA regulations.

Pet-Friendly Policy

Yes, we are a pet-friendly community. Residents must be able to care for their pets independently.

Covid and Flu Vaccinations

We provide Covid and flu vaccinations via our Medical Director and Williams Bros Pharmacy.

Aging in Place

Yes, residents can age in place at our facility. We currently have a waiting list, with typical availability within 3-45 days.
