Could you share pricing and any other general information?

Semi: $425.00
Small Private: $440.00
Large Private: $460.00

Could you provide an overview of the types of professionals on your staff and the training they undergo at your facility?

We have a Medical Director that oversees care. Two Nurse Practitioners, Registered Nurses, Licensed Practical Nurses, Certified Nursing Assistants that provide all care to all of our residents.
We have a full-time activities department, dieticians, social work, therapy (Occupational, Physical, Speech, and Respiratory therapist).
All staff receive training upon hire, yearly updates as well as educational training throughout the year based off new information and needs to maintain up-to-date certifications/qualifications.

Could you tell me about the measures you have in place to ensure the safety and well-being of your residents?

We diligently make sure to have appropriate staff/resident ratios as well as appropriate safety equipment as determined by nursing and therapy for all of residents on an individual/case by case need.

Do you offer any resources or support services to aid residents' transition into the community?

Each department works as a team with the resident/family to make sure a safe/appropriate discharge plan is in place. Our social work department drives this endeavor for each resident.

Do you offer transportation services for residents?

Transportation for medical appointments is provided on an as needed basis as well as when available.

Could you tell me a little about your dining experience?

Meals are provided in our large dining area and for those that choose and are able to dine independently in room. We encourage everyone to dine together to help provide interactions and a nice dining experience. We are able to accommodate many different types of needs (i.e. lactose free, gluten free, vegetarian, altered diets, thickened liquids, etc.).

What is the process that takes place if a resident has a complaint or an issue they want addressed?

We have a grievance policy in place where residents/families can complete a form and social work will drive this to make sure it is followed up on in a timely manner. A phone call to the facility to discuss concerns is also appropriate as well.

What is your policy on communication with resident's families? How do you keep them updated about their loved ones' well-being?

We make sure to have contact information updated in our system so we can contact the primary family contact with any changes or updates as they come. Family meetings are held within the first couple of weeks upon admission and then occur quarterly and as needed.

Do you all offer short-term or respite stays? How much does this cost?

Respite stays are covered privately. (Please refer to the private rates as listed above).
Short-term stays are typically covered under the person's insurance as they are usually admitted from a hospital. If the insurance does not cover, then it would also be a private pay scenario.

Are you all a pet-friendly community?

We are a pet-friendly community. Families wishing to bring in pets just need to provide current vaccination records.

Are residents able to get Covid vaccination and flu vaccinations in the community?

We offer the covid/flu vaccination for residents in the facility.

Are residents able to age in place?

Yes, if a resident comes in short-term and it is deemed unsafe for them to return to the community, we make every attempt to age in place if appropriate for the resident. We do not have a wait list currently.
