Our long term care, private pay rates are:
  • $335 a day for a private room
  • $325 a day for a semi-private room
This pricing covers room and board.

Facility Overview

We are a 76-bed facility, however, our building is split into 4 smaller houses comprised of 18-20 residents, offering a smaller facility feel.

Our Team

We have an administrator who is also a registered nurse and former director of nursing. Our team includes a Director of Nursing, Assistant Director of Nursing, unit manager, medical records staff, social worker, activities director, therapy department, restorative aide, registered nurses, licensed nurses, certified medication aide, and certified nurses aide. Additionally, we have a busy business office manager, a medical director, and a physician assistant available Monday through Friday. Many of our staff hold degrees and have received various training pertinent to their positions.

Security and Safety Measures

Our facility features locked entry doors on each unit that require a door code to enter and exit. We have surveillance cameras on the units and throughout the facility to ensure safety. Residents are frequently rounded on, and we have processes and interventions in place for falls, elopement, and infections. We provide equipment for safe transfers and use electronic charting to enhance medication administration safety. An auditing system for quality measures is also in place.

Resident Support and Resources

We have a social worker with access to many resources. There is a resident council, and we provide a chapel with access to outside Chaplin. Our leadership team and staff strive to improve lives and exceed expectations. The transport department schedules appointments and transportation for residents.

Homely Environment

Each house within our facility has its own kitchen and cook to provide a home-like atmosphere. Our dining area, central to our unit, is where residents gather, similar to being at home. We also serve residents in their rooms if they prefer.

Addressing Concerns

We take concerns from residents and families seriously. The manager and director investigate these concerns and work to resolve and improve them swiftly. We follow the guidelines of state and federal regulations and self-report all serious issues. Social services keep records as per state regulations.

Communication with Families

It is our policy to notify families of any changes in conditions, falls, weight loss, wound or skin concerns, new medications, and appointments. This communication is handled by clinical staff. Families are also included in care plan meetings.

Services and Policies

We offer short-term and respite care at the cost of $335 a day for room and board. While residents are not allowed to have pets, visiting pets are welcome. We do not have a waiting list for admission. We also offer and encourage Covid, flu, and pneumonia vaccinations. Residents are allowed to age in place in our facility.
