Professional Staff at Autumn Care of Saluda
Autumn Care of Saluda has a wide range of professionals on staff. Our administrator has 30+ years of administrative experience. The Director of Nursing has been at Saluda for nearly 5 years and is an RN. Other staff members include Physical Therapists, Occupational Therapists, Speech Therapists, RNs, LPNs, and Certified Nursing Assistants. In addition, the dietary department is managed by a Certified Dietary Manager and our Activity Director is certified. All of our staff members are required to complete in-service education each month assigned by our Corporate offices. Additionally, many of the professionals (RNs, LPNs, CNAs, and therapists) are required to complete continuing education courses.
Safety and Well-Being Measures
We have several measures in place to ensure the safety and well-being of the residents. In order to get into the building, there are codes on each of the doors; visitors can access the facility by ringing the doorbell. Additionally, each weekday we have a morning meeting and a clinical meeting where we review residents with any change in condition.
Transition Into the Community
We do offer transition into the community once the resident has completed their rehab. We have what we call the PATH Program (Progressive Approach To Home). This program starts in our first meeting with the resident and their family to see what the goals are, ascertain their living situation and then work with the resident to achieve this goal. Additionally, upon discharge, our Social Services Director will work with the family and resident to determine what needs and equipment they may need for home and what home health they would want to work with. Our goal for discharge is to have the resident be successful once they are discharged to home.
Transportation Services
We do offer transportation services to and from appointments.
Dining Services
Dining services are managed by our Certified Dietary Manager. Residents have a choice of eating in their room or they can eat in the dining room. Additionally, their meal selections are based on their likes and dislikes so we do offer meal choices to the residents.
Grievance and Feedback Process
Regarding complaints, we have a grievance process in place whereby a grievance form is completed by the resident/family, or they can be completed by a staff member on behalf of the resident. These are then forwarded to the appropriate department for follow-up and closure. Additionally, monthly we have a resident council where the residents meet to discuss the facility and any issues that might arise. Our administrator also has an open-door policy where you do not need an appointment to see him. He also rounds the building several times a day to ensure that the resident's needs are being met. Additionally, department heads are assigned a specified number of rooms for their concierge rounds in which they go to their rooms daily, speak with the resident and check to ensure that the rooms are up to our standard.
Family Communication
Communication with families is through the care plan meetings, as well as the PATH meeting that was mentioned earlier. Family members can also call to get an update on their loved one.
Short-Term and Respite Stays
We do offer short-term as well as respite stays. The rates for a respite stay would be our private pay rate. For short-term rehab, it would depend on the individual's insurance or if they have Medicare.
Pet-Friendly Community
We are a pet-friendly community and encourage pets to visit as we believe this is part of the person's rehab. Pets must be current on their vaccinations. They can visit, but they cannot spend the night.
Availability and Waitlist
We do not currently have a waitlist as we have beds available.
Vaccinations
Yes, we offer the COVID and flu vaccine here at the facility and encourage the residents to get both.
Aging in Place
Yes, residents are able to age in place. If they have completed their rehab and cannot return home, they can stay at Autumn Care of Saluda. We accept Medicare/Medicaid and most insurances.
