Autumn Care is dedicated to providing the highest quality of care to all its residents, guided by a mission to serve with compassion and commitment. As a skilled nursing facility, Autumn Care offers a range of services including therapy services, long-term care, and a unique housing option for the aged (HSI) that resembles assisted living but within a skilled care context.

Admissions and Care Types

The facility does not require residents in the HSI section to have a skilled need, allowing for private pay or the use of long-term care policies. It is important to note that pets are not allowed within the facility. Residents typically do not need to wear pendants for assistance.

Facility Size and Staffing

Autumn Care is an 80-bed facility staffed with a dedicated team of RNs, LPNs, CNAs, and MedTechs providing 24-hour care. Staffing levels are designed to ensure comprehensive care, with eight CNAs on the first and second shifts, and six staff members on the third shift, including two CNAs and a nurse on each side of the building.

Transportation and Activities

The facility offers a van for transportation, catering to residents' needs for doctor's appointments and other outings. Some residents enjoy simply riding along with the driver for leisure.

Dining Experience

Autumn Care provides a main dining room where residents can enjoy breakfast, lunch, and supper. The facility accommodates dietary needs and offers a select menu for those who do not require a modified diet, allowing residents to choose their meals from alternatives available for each mealtime.

Waitlist and Communication

There is currently a waiting list for admission to Autumn Care. The facility ensures open communication with family and friends of residents through a website, a Facebook page, and newsletters, keeping everyone informed and connected.
