Overview of Our Staff and Training
Our dedicated team comprises:
- Community Director, Cindee: 40+ hours a week (on call 24/7)
- Regional Director of Nursing, Dolores- Registered Nurse: 40+ hours a week (on call 24/7)
- Medication Aides: Certified through UW- Green Bay and on the state registry to pass medication. They undergo a delegation process by our community RN and receive on-the-floor training.
- Resident Assistants (Caregivers): Certified through UW- Green Bay and on the state registry, followed by on-the-floor training.
- Maintenance Director: 40+ hours a week
Safety and Well-being Measures
Each resident has an Individualized Service Plan (ISP/Care Plan) detailing their specific needs, ensuring tailored care and safety measures.
Transition Support and Resources
While we donβt offer specific resources for transition, we are open to discussing and addressing any particular needs.
Transportation Services
Our bus is primarily for outings, but we assist in coordinating cab services for residents' needs, with costs being the responsibility of the resident/family.
Dining Experience
Our menus, created by Martin Bros. in accordance with a dietician, are served in our dining room where staff take orders and serve meals directly to residents.
Addressing Complaints and Issues
Residents/families can fill out a grievance form or verbally express concerns, which are then followed up by our Community Director.
Cost of Living
A cost sheet is available upon request for detailed information on living expenses within our community.
Communication with Residents' Families
Communication is primarily with activated POAs, or as designated by the resident through a signed form.
Short-term or Respite Stays
We offer respite stays starting at a minimum of two weeks, with costs of $300/day for Assisted Living and $350/day for Memory Care.
Pet Policy
We are no longer a pet-friendly community.
Availability and Waitlist
While we currently do not have a waitlist, our spaces are filling up quickly, with limited units available in memory care and assisted living.
Vaccination Services
Guardian Pharmacy provides yearly flu clinics and has offered COVID and Shingle vaccinations during those clinics.
Aging in Place
Our community supports aging in place with both assisted living and memory care options, subject to certain care limitations.
