• 100 Hall Semi-private Rehab stay: $290 per day
• 100 Hall Private room Rehab stay: $311 per day
• 200 Hall Rehab semi-private: $288 per day
• 300-400 Hall Long term care semi-private room: $278
• 300-400 Hall Long term care private room: $286

Staff Composition

Our team consists of RN/LPN/STNA, LSW, Dietary staff, LNHA, and Activities Recreational staff.

Social Work and Discharge Planning

The Social Work department is responsible for setting up discharge planning, which includes arrangements for community needs, Durable Medical Equipment (DME), Home Health Care (HHC), Meals on Wheels (MOW), Passport services, and assistance with applying for Medicaid.

Transportation and Billing

We do not provide transportation services, and any transportation required is billed to the patient.

Dining Arrangements

Currently, dining is confined to patient rooms to ensure safety and comfort.

Addressing Concerns

Concerns are addressed by the LNHA, LSW, DON, and we also offer ombudsman services for additional support.

Care Coordination and Meetings

A care coordination meeting is scheduled within 5 days of admission to initiate discharge planning. For long-term care (LTC) residents, care plan meetings are held quarterly.

Respite Stays and Payment

Respite stays are private pay at $290 per day unless covered by Medicaid. We coordinate with the Area Agency on Aging for level of care assessments.

Vaccinations

COVID, Flu, and Pneumonia vaccinations are available to our residents.

Pet Policy

Pets can visit with proper documentation of shot records, but cannot reside permanently within the facility.

Aging in Place

Patients can age in place as we are a nursing home, not an assisted living facility. We do not have a waitlist and currently have bed availability for both short term rehabilitation and long term care. Please note, all of our rooms are shared, with the few private rooms currently occupied.
