Skilled Nursing Facility Room Rates

Our Skilled Nursing Facility offers the following room options:

• Semi-private room (shared room): $420 a day
• Semi-Deluxe (shared room): $440 a day
• Private room: $475 a day

We require a 1-month security deposit and 1 month's room and board upon admission.

Our Facilities

We have our Independent Living building (Stephanie is the property manager-847-961-7699), Memory Care (Terrance Dyson 847-961-7600) and then our Skilled Nursing Facility.

Admission Process

We do have a waiting list for Long Term Care at both our Memory Care and Skilled Nursing Facility. In order for an individual to be considered, we would need a History and Physical from the Doctor, as well as, a current medication list to make sure we can clinically accommodate the individual’s needs. That would need to be faxed in at 872-469-1791.

Skilled Nursing Facility Pricing

I can give you pricing for our Skilled Nursing facility: our Semi-private room (shared room) is $420 a day, Semi-Deluxe (shared room) $440 a day, and our private room is $475 a day.

Activities Department

We have an excellent Activities department which offers many activities to get our residents involved and get to know their peers. They do go on outings once a month into the community, whether it’s a trip to Walmart, the movies, etc. our Activity bus will take them.

Transportation

Unfortunately, we do not offer transportation from Alden, however, we do go through 2 transportation companies if individuals need to go out to a doctor appointment.

Dining Experience

We offer a restaurant style dining experience. Our dining staff will come into the residents' rooms to take their orders for each meal service at that time they will let the staff know if they will be coming to the dining room for their meal. If they do not want to come to the dining room the meals are brought to the individual's room.

Addressing Complaints

If the resident has a complaint they need addressed, we ask that they let the nursing staff or management know so the matter can be taken care of right away.

Respite Stays

We do offer respite stays and the cost is our daily room rate. We would require payment in full upon admission. With that, we would need the History and Physical and medication list as well.

Pet Policy

We do allow pets to come into the facility to visit their loved ones as long as they are potty trained.

Vaccinations

Patients are given the opportunity to get their COVID/flu vaccinations here at the facilities.
