ActivCare's Commitment to Memory Care

ActivCare has a rich history in Memory Care, choosing to schedule a licensed nurse 24/7, alongside our Program Lead who is a licensed nurse on staff Monday-Friday. Our team includes CNA’s, Med Techs, and Caregivers, all of whom undergo 40 hours of Dementia training in addition to ongoing required training. New care staff receive a comprehensive 2-week on-the-floor training.

We pride ourselves on our experienced care staff, some of whom have been with ActivCare since its opening in 2016, while others have been with the organization at other locations for over 20 years.

Design and Environment

All ActivCare Locations are designed equally and intentionally, providing a secure environment with an open floor plan that enables residents’ freedom of movement within the community, as well as outdoors in a secured manner. All paths lead back to the large great room where residents gather for daily meals, exercise classes, music performances, and hands-on activities. Wide doorways and corridors facilitate easy access and encourage social interaction.

Comprehensive Assessment and Care Planning

Joan Tincher, RN, MSN, GNP, Care Manager and Geriatric Nurse Practitioner, performs a comprehensive assessment for prospective residents. With over 10 years of leadership at ActivCare, she guides families through the complexities of transitioning a loved one to their new home, ensuring a care plan individualized to each resident.

Medical Services and Outings

ActivCare offers outings for residents, including scenic trips and picnics. Additionally, we provide an in-house medical team from Pine Park Health, offering services such as primary care visits, x-rays, blood draws, UA testing, dentistry, psychiatry, and podiatry through most insurances.

Continuum of Care Levels

We offer a continuum of care levels to accommodate the progression of Dementia. Our Club Level dining and care levels are designed to meet the varying needs of our residents, from those who are independent to those who require more assistance with daily activities.

Communication and Pricing

Communication with family members is primarily through the nurse on duty, with the option for families to call or visit at any time. Our pricing starts at $8,250.00 per month, up to $11,750.00. Respite stays are available with a minimum of 30 days, and pricing can be discussed during a tour or conversation.

Pet Policy and Availability

Family members may bring pets to visit, but residents are not allowed to have pets live with them. We do not have a waitlist, but availability may vary based on the level of care needed and room preferences. Currently, we have 4 rooms available.

Feedback and Concerns

Residents or family members can address concerns directly to our Program Leader or Administrator, with the local Ombudsman available for those who prefer to speak to someone outside the community. All issues and complaints are quickly addressed.
